Community Care Network (CCN)

The VA determines a veteran’s eligibility for CCN benefits. Once approved, a VA case manager referral through the Homemaker/Home Health Aide Program (HHHA) leads to an evaluation to set the care plan and allocate home care hours. Enhanced In-Home Support for Veterans and Spouses
